Good attendance
Applicants must maintain a good attendance record and provide an attendance record or school verification.
Hurricane impact
The student’s schooling, home, family, or community must have been materially impacted by a hurricane.

Current enrollment in a recognized school or educational program
Recent transcript/report card showing a cumulative GPA of 3.0 or higher
Attendance record or school letter confirming good attendance
Brief statement describing the hurricane’s impact and the student’s educational need
Proof of enrollment and supporting hurricane-impact documentation when available
Parent/guardian information and consent for applicants under 18
